Anesthesiologist -- Dr. Steven Shafer Testifies



Dr. Steven Shafer -- the prosecution's final witness -- will wrap up his testimony today from last week.

Dr. Shafer is a Propofol expert, and he's expected to testify that Murray was criminally negligent in the way he treated Michael Jackson.

* Thursday, Shafer told prosecutors he was hired 20 years ago by the company that produces Propofol to calibrate the correct dosing for the drug.
* Shafer said the exact dosage of Propofol is crucial -- because even a slight discrepancy could be the difference between a patient sleeping for a few minutes, and one sleeping for a several hours.
* Shafer told prosecutors that thanks to Murray, patients ask him every day whether he's giving them "the drug that killed Michael Jackson."
Shafer showed the court how Propofol is extracted out of the bottle with a syringe ... saying air needs to be drawn first to enable drug to enter the syringe. This could take a few minutes.
* Prosecutors are showing the jury a video on how to safely administer Propofol. Dr. Shafer told the jury the key is making sure the flow of oxygen should never be interrupted.
* Infusion of drugs should always be done through a pump to avoid overdosing.
* Shafer makes a point it take a while to get the pump ready, saying "this takes time and care to do this correctly."
* Shafer says a doctor must get an "informed consent" from the patient ... this is not just a piece of paper but a process in which the doctor informs the risks, benefits and alternatives to the patient. That's when the patient signs the document that proves the process was followed. He said a verbal informed consent is NOT binding.
* Before giving the Propofol, doctors do an "anesthesia pause" -- one last check to make sure everything is right.
* Shafer said the anesthesiologist should be close to the patient at all time.
* As part of their job, they also take copious notes to record everything that happens in the OR.
* If the monitors show an obstruction to the airway, all doctors need to do is lift the chin slightly. If the chin lift doesn't work, doctors can grab the jaw and move it forward... all simple maneuvers.
* If the patient is still not breathing docs can force oxygen into the patients lungs with an ambu bag.
* In an event of a cardiac arrest, the anesthesiologist should immediately call for help.
Shafer's key points:
- Infusion of drugs should be done ONLY through a pump to avoid overdosing.
- "Informed consent" is not just a piece of paper but a process in which the doctor informs the patient of all the risks, benefits and alternatives.
- A verbal consent is NOT binding.
- Record keeping is paramount
- If the patient stops breathing, the first thing a doctor should do... call for help.
* Shafer told prosecutors guidelines should be strictly followed even when administering "a little bit" of Propofol because every patient responds differently to the drug. He said the worst disasters occur during sedation ... when doctors "cut corners." He told the court none of the safeguards were in place when Murray gave MJ Propofol.
* Shafer said Murray's treatment of MJ is both an "egregious" and "unconscionable" violation of standard care.
* He called 15.5 liters of Propofol Murray bought -- about four gallons worth -- "an extraordinary amount" for one person.
* The fact that Murray never kept any records is "unbelievable," according to Shafer.
* He said Murray "left the steering wheel" when he went to bathroom to relieve his bladder.

* Shafer told prosecutors there are 2,500 articles about Propofol sedation... and only one -- very flawed -- article about Propofol and insomnia. He said, "We're in a pharmacological Never-Never Land. It's only ever been done to Michael Jackson in history as far as I know."

Dr. Steven Shafer - ultima testimonianza della pubblica accusa - concluderà oggi la sua testimonianza della settimana scorsa.

Dr. Shafer è un esperto di Propofol, e lui si aspetta di testimoniare che Murray è stato criminalmente negligente nel modo in cui trattava di Michael Jackson.

* Giovedi, Shafer ha detto ai pubblici ministeri che è stato assunto 20 anni fa dalla società che produce Propofol per calibrare il corretto dosaggio del farmaco.
* Shafer ha detto che l'esatto dosaggio di Propofol è di fondamentale importanza - perché anche una leggera discrepanza potrebbe fare la differenza tra un paziente addormentato per qualche minuto, e una zona notte per diverse ore.

* I procuratori stanno mostrando alla giuria un video su come amministrare in modo sicuro il Propofol. Dr. Shafer ha detto alla giuria che la chiave è fare in modo che il flusso di ossigeno non dovrebbe mai essere interrotto.

* Shafer ha detto che l'anestesista dovrebbe essere vicino al paziente in ogni momento.
* Come parte del loro lavoro,è necessario anche prendere appunti per registrare tutto ciò che accade in sala operatoria.
* Se i monitor mostrano una ostruzione alle vie aeree,quello che tutti i medici devono fare è alzare il mento leggermente. Se l'ascensore non funziona, i medici possono afferrare e spostare la mandibola in avanti ... tutte le manovre semplici.
* Se il paziente non sta ancora respirando docs può forzare l'ossigeno nei polmoni dei pazienti con un pallone Ambu.
* In caso di arresto cardiaco, l'anestesista dovrebbe immediatamente chiedere aiuto.
Shafer punti chiave:
- L'infusione di farmaci dovrebbe essere fatta solo attraverso una pompa per evitare un sovradosaggio.
- "Il consenso informato" non è solo un pezzo di carta ma un processo in cui il medico informa il paziente di tutti i rischi, i benefici e le alternative.
- Il consenso verbale non è vincolante.
- Registrazione è di primaria importanza
- Se il paziente smette di respirare, la prima cosa che un medico dovrebbe fare ...e far partire una richiesta di aiuto.
* Shafer ha detto ai procuratori che linee guida dovrebbero essere seguite scrupolosamente, anche quando si somministra "un po '" di Propofol perché ogni paziente risponde in modo diverso al farmaco. Ha detto che i peggiori disastri si verificano durante la sedazione ... quando i medici usano "scorciatoie". Nessuno ha detto al tribunale delle garanzie che erano al loro posto quando Murray ha dato MJ Propofol.
* Murray Shafer ha detto che il trattamento di MJ è sia un "egregio" e luna violazione "irragionevole" di cura standard.
* Ha chiamato i 15,5 litri di Propofol che Murray ha acquistato - circa quattro litri vale la pena - "una quantità straordinaria" per una persona.
* Il fatto che Murray non ha mai mantenuto alcun record è "incredibile", secondo Shafer.
* Ha detto che Murray, "ha lasciato il volante", quando andò al bagno per alleviare la sua vescica.

Shafer ha detto Ai pubblici ministeri ci sono circa 2.500 articoli sulla sedazione da Propofol ... e solo uno - molto imperfetto - articolo su Propofol e insonnia. Ha detto: "Siamo in un farmacologico Never-Never Land. E 'sempre e solo stato fatto per Michael Jackson nella storia per quanto ne so".
